Policy
Hibu does not allow “Interested in Services” drop-down fields or open “Message/Comments” text fields on any form for websites associated with medical, healthcare,
or insurance services.
This rule exists to prevent collection of protected health information (PHI) on our websites.

What This Policy Requires
- No Services Drop-Down
- Forms must not include any field that allows a visitor to select or describe medical, health or insurance services.
- No Message or Free-Text Field
- Forms must not include open comment boxes where visitors could disclose protected or sensitive information (e.g., symptoms, diagnoses, medical history, insurance status, etc.).
- Permitted Fields
- Only non-sensitive, structured fields may be used, such as:
- Name
- Phone
- Preferred appointment date/time
- Preferred contact method
